What to bring
To help us better serve you, please bring the following:
Basic Information
1) Documentation of your insurance coverage (card, etc)
2) Your driver's license or identification
3) All past medical records and imaging (mammograms, ultrasounds, etc) that are related to your breast health.
4) If you have currently and/or previously been diagnosed with cancer , please bring copies of all pathology reports, operative reports, previous oncology treatment records.
5) Current medication list with doses.
6) Download and complete the following forms regarding our policies:
Abnormal Mammograms
If we are seeing you due to an abnormal mammogram, please bring that mammogram and the radiologist report.
Second Opinions
For a second opinion, we need all records from physicians who were party to the diagnosis you wish us to validate.
