We ask that all NEW patients bring:

In addition:

  • A copy of your referral, if you were referred to us by another physician or your insurance requires it in writing.
  • Download and complete a copy of our Breast Health Intake Form
  • You are more than welcome to bring a spouse or a friend for comfort; however, they may not be able to accompany you during certain parts of your the exam.
